The presentation describes a complex clinical case of antibody-mediated rejection following kidney transplantation. It follows the patient’s course from transplantation through the development of rejection, diagnostic findings including biopsy and donor-specific antibodies, and the applied treatments such as immunoadsorption, thymoglobulin, rituximab, and alemtuzumab. The case highlights the challenges of management, partial therapeutic response, and the clinical questions raised during treatment.
