The publication examines the use of intravenous versus subcutaneous recombinant erythropoietin in hemodialysis patients, with particular attention to the effect of obesity. The treatment of anemia associated with chronic kidney disease is an important part of dialysis care, and erythropoietin preparations play a central role in this process. However, the route of administration may influence the required dose, treatment efficacy and practical use.

The study was based on a dialysis cohort from the southeastern United States. The authors analyzed how the dose requirement for recombinant erythropoietin changed when treatment was switched from intravenous to subcutaneous administration, and whether body mass index had an effect on this response. Hemoglobin levels, weekly erythropoietin dose, iron status parameters and the degree of obesity were also included in the analysis.

The results showed that hemoglobin levels remained generally stable during the observation period. In the overall patient group, the weekly erythropoietin dose decreased, although the independent effect of obesity was not clearly confirmed in multivariate analysis. The practical significance of the publication is that it highlights how the route of erythropoietin administration, body weight, iron status and individual dose response may all influence treatment decisions in dialysis patients. In obese patients, regular monitoring of hemoglobin targets, drug dose and clinical response is therefore especially important.
